  • Specific deductibles beginning at $20,000.

  • Retirees under age 65 can comprise up to 15% of the group.

  • Minimum 75% employee participation (excludes waiver cards).

  • Excluded groups: MEWAS’s, Trade and Fraternal Associations, Employee Leasing Firms and Indian Tribes.

  • For groups who offer HMO options, at least 50% of the employees must be enrolled in the self funded program.


Available options:

  • Aggregate coverage.

  • Monthly aggregate accommodation.

  • Specific advance funding.

  • Aggregating specific deductibles.

  • Variety of contract options are available (24/12 with no change in administrators).

  • Specific lifetime maximums up to $5 million.
